Job Summary
This is a challenging and rewarding role that requires strong leadership skills, clinical knowledge, and a passion for delivering high-quality care to residents in our aged care community. The successful candidate will be responsible for leading the care team, ensuring the delivery of person-centred care, and promoting a culture of safety and excellence.
Key Responsibilities
* Care Service & Delivery:
o Provide clinical and care leadership with a person-centred approach to enable residents and their families to experience a quality service.
o Assist the General Manager in ensuring Opal's policies and procedures are followed and appropriate training and resources are provided to the team to deliver high-quality care in line with Opal standards.
o Responsible for reviewing all new admissions, significant changes to resident condition, and residents changing trajectories to determine the presence of conditions that may impact the care and health of the resident.
o Proactively manage risk in the care community and consult with the General Manager to implement measures to prevent/mitigate risks.
o Ensure Intentional Rounding is a core activity in the delivery of care.
o Engage with residents and their families, acknowledging and acting on feedback in a timely manner to resolve concerns, address issues, and build trust and confidence in our team and the care and services provided.
o Ensure the care community is always compliant with accreditation standards and continually looks for opportunities for improvement.
o Proactively track and analyse clinical indicators to identify areas of risk in the care community and implement measures to prevent/mitigate risk.
o Support the General Manager in developing/role modelling a customer service mentality that places the resident and their relatives at the centre of everything we do.
o Support the creation of a physical environment that is clean, resourced, and well-maintained to provide residents with a home-like environment.
o Use evidence-based practice as a path to inform clinical decision-making.
* Workforce:
o Actively monitor and review skill mixes in the care community to ensure skills and knowledge complement each other to deliver high-quality care to residents.
o Continually support the development of Registered Nurse and AIN talent in the care community.
o Provide timely, consistent, and constructive coaching to the team to facilitate high-functional teams.
o Ensure the team receives orientation, performance appraisals, and learning and development programs in line with Opal standards.
o Contribute to reward and recognition activities to reinforce positive behaviours that deliver resident outcomes.
o Identify gaps in individuals' skill/knowledge and collaborate with the General Manager in implementing appropriate development programs. Address performance issues with a learning and supportive approach.
o Develop and/or implement Opal programs for mentoring student placements and new graduates.
* Safety and Compliance:
o Support the creation and sustaining of a safety culture where there is zero harm to the team and residents.
o Adhere to the requirements of the Master Emergency Management Plan.
o Promote health and safety in the care community and be a role model for safety by conducting safety walks, investigating incidents, and sharing lessons learned.
o Ensure all care equipment is in place to provide a safe working environment.
Requirements
* Essential Criteria:
o Current Practicing Registration as a Registered Nurse with AHPRA for a minimum of 3 years.
o Current and suitable police check.
o Experience leading and managing care team members.
o Knowledge of aged care industry, regulations, and funding (AACQA; ACFI).
o High level of computer systems proficiency.

o Relevant higher education, qualification in the field of gerontology or similar, or progress towards same.
o Understanding of Workplace Health and Safety framework and regulations.
o Residential Aged Care experience.
