This may be a nursing home, residential service, their own home and/or the carer’s home.Tasks include:Deliver high quality assessment, consultation, care planning and intensive service delivery to Aged Psychiatry identified clients.Screen referrals, perform an initial assessment over the phone or face to face and provide information to clients, clinicians & service providers.Determine eligibility/urgency for the appropriate community and ambulatory services.About the cultureYou will have the opportunity to be part of a highly skilled, professional and dedicated team who take pride in providing high quality services.
