Job Overview
Clinical Case Workers are integral members of our multidisciplinary team, delivering mental health and social support services to individuals in need. Key Responsibilities
* Develop and implement individualised care plans to address the unique challenges faced by clients
* Consult and liaise with the team and Team Leader in the development and implementation of health promotion activities
* Engage and liaise with local GP practices, psychiatrists, psychologists, psychosocial and allied health services as well as internal services using the stepped model of care provided
* Maintain accurate and up-to-date client records and contribute to the team network
Required Skills and Qualifications
We are seeking a Clinical Case Worker who possesses:
* Relevant experience in community services;
* Social work Degree or equivalent;
* 2-5 years' experience in community settings;
* Care coordination and/or case management experience
* Demonstrated experience in delivering mental health and/or social support services, preferably in a community-based or primary care setting;
* Strong clinical assessment and case management skills, with the ability to work collaboratively within a multidisciplinary team;
* Familiarity with relevant legislation, regulations, and best practices in the healthcare and social services sectors.
