Job Opportunity:
We are seeking a dedicated and compassionate Disability Support Professional to join our team. The successful candidate will be responsible for providing personalized care and support to individuals with disabilities, helping them to achieve their personal goals and enhance their quality of life.
* Personal Care: Assist clients with daily living activities, including bathing, dressing, grooming, and toileting. Provide support with mobility and transfers, ensuring clients' safety at all times. Administer medication as prescribed and monitor clients' health conditions.
* Daily Living Support: Help clients with meal preparation, feeding, and household tasks. Assist clients in participating in social, recreational, and community activities. Encourage and support clients to develop skills and achieve their personal goals.
* Health and Safety: Monitor and report any changes in clients' health and well-being. Ensure that clients' environments are clean, safe, and well-maintained. Adhere to all health and safety protocols and procedures.
* Documentation and Communication: Maintain accurate and up-to-date records of clients' care and progress. Communicate effectively with clients, their families, and other healthcare professionals. Participate in care team meetings and contribute to the development and review of care plans.
