About us:
Transition Care Program provides short-term care aimed at optimizing functioning and independence of older people after hospital stays. It is goal-oriented, time-limited, and therapy-focused, offering services including physiotherapy, occupational therapy, social work, nursing support, and personal care. The goal is to enable older people to return home rather than prematurely entering residential care.
About the role:
* Manage cases for clients in the community rehabilitation program.
* Provide clinical rehabilitation therapy to clients on the community rehabilitation program.
* Deliver specific physiotherapy or occupational therapy activities within the multidisciplinary team.
* Supervise, support, and delegate therapy programs to the allied health assistant.
* Participate in quality activities within the team.
About you:
Applicants must be registered physiotherapists or occupational therapists, maintaining registration with AHPRA and the Physiotherapist/Occupational Therapy Registration Board of Queensland, including any endorsements.
What we offer:
Total remuneration of up to $132,567 per annum, comprising:
* Hourly rate between $43.68 and $58.59
* 12.75% employer superannuation contribution
