Position Overview
Carer Hub Navigator – full time, Toowoomba QLD. This role integrates community development and engagement to raise awareness and enhance accessibility to Carer Gateway services. The Navigator will act as the welcoming face and guiding hand for carers in our new Carer Hubs.
Responsibilities
* Build and maintain strong, sustainable relationships with community organisations, health services, schools, aged care facilities, and local government agencies.
* Provide seamless support to carers through a 'no wrong door' approach, acting as a consistent presence and reliable supporter.
* Actively promote and facilitate social interaction, wellbeing, and a sense of belonging within the Carer Hub.
* Advocate and connect carers to vital community resources, cultivating meaningful relationships that support wellbeing and independence.
Qualifications
* Experience working with family carers in mental health, alcohol and other drugs, and community support services.
* Certificate IV or higher in a support discipline, or equivalent community sector experience.
* Current valid driver's licence.
* Strong technology skills and appropriate IT skills.
* Satisfactory national police records check (within 12 months).
* WWCC / QLD Blue Card.
* NDIS workers screening check / QLD Yellow Card.
* Right to work within Australia.
* NDIS Worker Orientation Module Certificate.

* Salary SCHADS 4B ($43.08–$46.35 per hour) plus super.
