Albury Wodonga Aboriginal Health Service is excited to announce a rare opportunity to join our innovative Pathway to Good Health program. We are seeking a passionate Health Navigator to deliver child and young person‑centred care, supporting Aboriginal children in out‑of‑home care to access timely, culturally safe health assessments, referrals, and wrap‑around care.
This role requires collaboration with the PTGH team members, VACCA, Dept Communities and Justice, carers, and service providers to deliver quality, culturally sensitive care plans. The successful candidate will advocate for families, ensuring respectful and inclusive service delivery. They will act as a critical bridge between families, carers, and health systems to empower children and young people. Direct service provision, strategic service planning, and administration are also key responsibilities of this role.
The ideal candidate will have qualifications in nursing, social work, or Aboriginal health practice, and experience working with children and young people (0–17) and knowledge of child protection or out‑of‑home care systems. Strong communication, problem-solving, and interpersonal skills are essential for success in this role.
