Support Specialist - Community Care
We are seeking an experienced Support Specialist to join our team and provide vital support services to vulnerable members of our community.
This role will involve conducting face-to-face assessments for clients referred via the Integrated Assessment Tool (IAT) and Aged Care Assessor application, identifying their needs, establishing goals, and developing tailored support plans connecting them to relevant Commonwealth and community services.
You will have a strong understanding of the aged care sector, including My Aged Care system usage, and be able to work collaboratively with diverse stakeholders to achieve positive outcomes.

About You
To succeed in this role, you will need:
* A background working with diverse and complex needs, such as frail aged, dementia, or disability;
* Experience within the aged care sector, including My Aged Care system usage;
* Strong interpersonal capability, sound judgement, and a commitment to supporting older people and their carers;
In addition, you will bring essential skills including written and verbal communication, interpersonal skills, ability to work under pressure, and adherence to guidelines.
