What is our ServiceManager role?
This role provides strategic and operational leadership for theOlder Adult Community Integrated Care Hub (OA CIC Hub), overseeing a multidisciplinary team and working alongside medical specialists such as GPs and Geriatricians to deliver specialist-led community-based care for older adults with chronic and complexconditions.
Accountable for the effective management and utilisation of human, financial and physical resources relevant to the designated clinical and non-clinical areas to ensure the provision of safe,high quality and efficient patient care.
Builds partnerships with community services, identifying servicegaps, and ensures the Hub is community-oriented, responsive topatient needs and partnered with general practice and othercommunity-based providers.
