Role and responsibilities
The Connections Program aims to improve continuity of care for patients who are in public correctional centres with Drug and Alcohol problems and who are being released to the community. The Connections Program works within an assertive linkage model that links patients with relevant health and welfare service providers pre- and post-release. The Connections Transition Coordinator will utilise a broad array of contacts, both in the correctional and community environments, to link patients into services appropriate to their individual needs.
Salary & Benefits
Your salary ranges from $112,396 (Year 1) to $116,038 (Year 2) for Social Worker, Counsellor and Occupational Therapist classifications, or $77,358 (Year 1) to $113,226 (Year 9) for Psychologists, plus superannuation and leave loading. You’ll also have access to:

Vaccination: This is a Category A position; if successful you will need to provide proof of vaccination for measles, mumps, rubella, hepatitis B, varicella, diphtheria, tetanus, pertussis and influenza.
