The primary role of an Advanced Healthcare Provider is to deliver high-quality patient care services in a community-based setting. This position requires the ability to perform comprehensive assessments, provide acute and assertive outreach services, and support patients with complex needs.
* Main Responsibilities:
* Deliver high-quality patient care services in a community-based setting
* Conduct comprehensive assessments and provide acute and assertive outreach services
* Support patients recently discharged from inpatient services and provide care to those presenting with acute needs
Key Skills and Qualifications:
* Relevant tertiary qualifications as a Registered Nurse
* Current registration with AHPRA
* Demonstrated experience in the assessment and management of people presenting with acute mental health disorders
* Excellent negotiation skills and verbal and written communication skills
* Demonstrated knowledge of relevant legislation and policies
