About The Role
NCN Health is seeking a Case Partner to join our Support at Home Program team. The Care Partner supports older people to live safely and independently at home through person-centered care under the Support at Home Program. They coordinate services, explain the program, and help participants understand their goals, budgets, and support options. The role involves monitoring care plans and budgets, promoting independence, and addressing risks and changing needs. Working within the NCN Health team, the Care Partner ensures continuity of care, quality improvement, and regulatory compliance.
About You
* Vaccination against seasonal Influenza (mandatory)
* Vaccination against Covid-19 (recommended)
* A relevant post-secondary qualification in an appropriate Health and/or Human Services practice, including Community Services, Aged Care, Nursing, Allied Health, Social Work, Care Coordination.
* Demonstrated experience in assessment, care management, care coordination, or participant support roles, preferably in aged care, community health, or disability service context.
* Excellent interpersonal, communication, and advocacy skills, with the ability to build respectful relationships with participants, families, and service providers.
* Demonstrated high level of written and verbal communication skills with the ability to develop and maintain collaborative partnerships with internal and external stakeholders.
