* SHADS - Schedule B Level 5: $100,776 - $105,340
* Fixed Term Full Time role until 30/06/2029

POSITION DETAILS

The purpose of this position is to provide support to the Care Finder target population. The Care Finder program will provide specialist and intensive assistance to help people in the Care Finder target population groups, to understand and access aged care services and connect with other relevant supports in the community through a person-centred approach that maintains dignity, respect, choice, and self-determination in line with Care Finder Policy and guidelines.

RESPONSIBILITIES

* In consultation with clients, undertake holistic assessments and support planning to determine and document individual needs and access to supports that are client-led.
* Provide specialist and intensive assistance to help people in the care finder target population (senior Australians who need intensive support, who could otherwise fall through the gaps. to understand and access aged care services and connect with other relevant supports in the community.
* Ensure that individuals are valued and respected, and that support aims to promote independence and client self-determination.
* Assertive outreach: Attend meetings, build relationships with Aged Care providers and Community Providers, and engage clients to provide education around the Care Finder program.
* Facilitation of information sessions in the community to identify and reach vulnerable older people
* Meet with providers to arrange services, such as calling providers to check availability and attending meetings with providers and connect with other relevant supports in the community
* Engagement and rapport building with potential clients and other services.
* High-level check-in with clients on a periodic basis and follow-up support once services have commenced
* Support clients to understand and connect with required services, including My Aged Care, using warm referrals and monitor client engagement.
* Organise and participate in case conferences and interagency meetings in a collaborative and consultative manner, including mandatory participation in staff/team meetings and participation in community of practice meetings.
* Promote Footprints programs and services and advocate for service options for the target group, in the wider community, through sound working relationships with partners and networks within government, non-government and private sectors.
* Undertake administrative functions, including maintenance of client data base, maintaining a high standard of client files, reporting requirements, and data input activities.
* Participate in the professional support and development of the Care Finder Program and professional development.
* Contribute to the ongoing development and evaluation of program practice, ensuring continuous quality improvement and compliance with the Program Guidelines and the Aged Care Quality Standards.
* Provide culturally safe services
* Provide Case Studies as per contract requirements.
* Ensure client contacts and records are current and accurately maintained in the client management system
* In collaboration with the Program Coordinator, Care Finders and other relevant external service providers, support and monitor the appropriateness and responsiveness of support services
* Promote Footprints programs in the community through sound working relationships with service provider networks

General

* Follow all organisational policies and procedure
* Attend internal meetings relevant to the case manager position
* Participate in professional supervision sessions, and ongoing learning and development activities
* Promote and maintain a safe and healthy workplace, ensuring adherence to WHS policies and procedures
* Work within the vision, mission, and values of Footprints

KEY SELECTION CRITERIA

Essential

* Relevant Tertiary Qualification, e.g., Community Services, Human Services, Social Work, or related discipline and/or relevant experience
* Demonstrate local community connections with the care finder target population or a specific subgroup within the care finder target population
* A detailed understanding/ability to rapidly attain a detailed understanding of the range of aged care supports and services, and other relevant supports that are available in the local community, the process to access, and any eligibility requirements for other relevant supports in the community
* Demonstrated knowledge and experience in working with a diverse client group, including older people, people with disabilities, people experiencing mental illness, and people experiencing homelessness or those at risk of homelessness, and their carers/families.
* Demonstrated experience in community networking and working collaboratively with external service providers to plan and provide consistent, responsive, and appropriate client-focused services.
* Well-developed interpersonal, verbal, and written communication skills, and demonstrated ability to work effectively in a team environment.
* Adhere to client/staff confidentiality and consent when sharing information.
* Computer literacy and the ability to effectively manage a client database.
* Ability to work remotely from a team when required, and ability to utilise virtual meeting platforms e.g., Microsoft Teams, Zoom.
* Current class C drivers license
* Use of own comprehensively insured vehicle will be required (paid km's) where an office vehicle is not readily available
* Office vehicles are available in some locations
* Some travel through Care Finder delivery regions
* National NDIS Worker Screening Check or current Yellow Card
* Current First Aid and CPR Certificate
