About the role

A Support Coordinator plays an integral role in supporting participants, their networks and providers to understand and navigate the National Disability Insurance Scheme (NDIS). Support Coordinators assist participants to develop and implement their NDIS plans, access a range of community and disability services, establish and monitor supports to assist with daily life and develop their capacity over time to coordinate, self-direct and manage their own supports.

ConneXtions has a team of support coordinators who come from a variety of backgrounds including mental health, disability, early childhood, community services, youth work, disability, education and social work. As a Support Coordinator you will:

* Support participants and their families to understand and implement their NDIS Plans and the reasonable and necessary principles relating to plan implementation.
* Work with participants to explore and increase opportunities to connect with a range of community services and engage quality service providers.
* Work with the participant to strengthen and maintain informal support networks.
* Monitor funding and utilisation and maximise the value for money they receive from their supports.
* Work with participants increase their capacity to manage/direct their own supports.
* Prepare for NDIS planning meetings and gather evidence to support access to services
* Arrange for any assessments required to determine the nature and type of funding required
* Be the key contact for service issues, complaints, major changes and plan reviews. Assist participants to prepare for their plan review by supporting them to review goals and progress, assess whether they achieved their goals and got value for money for their plan, identify solutions to problems experienced in implementing the plan and consider new goals.
* Develop service agreements and create service bookings with preferred providers.

You will have

* Relevant tertiary qualification in a field such as social work, disability studies, or community services
* Minimum 1 years' experience in a similar support coordination or case management role
* Strong understanding of the disability support sector, including relevant legislation, policies, and funding schemes
* A commitment to the ConneXtions Code of Conduct including personal and professional boundaries
* Excellent interpersonal and active listening skills
* Ability to adapt communication style to meet people's needs
* Ability to resolve conflict and negotiate successful outcomes for people
* Strong administrative skills; time management, coordination of tasks, efficient work practices
* Ability to build and maintain strong community connections and relationships with service providers, individuals and/or families/carers
* Current Australian Driver's License, your own car (or access to one) with full comprehensive insurance
* Current First Aid and CPR Certificate
* Victorian Working with Children Check
* NDIS Worker Screening Check
* Completion of all requested onboarding and annual mandatory training
