* We provide services and supports to adults, and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection and referral services.
* As a Peer Support Worker - Journey to Social Inclusion this is your opportunity to utilise your personal experience and perspective of lived or living experience to support clients of the J2SI Program and break down the barriers of client engagement.
You will work collaboratively with Case Managers and other team members to support clients to identify and achieve set goals and outcomes.
Key Responsibilities:
* Use lived experience to build effective relationships and support client goals.
* Guide clients to connect with community, establish a home, build skills, receive emotional support and transition from homelessness to stable housing.
* Advocate for clients, identify risks, follow safety procedures and contribute to service improvements.
Maintain accurate client records, participate in meetings, support a positive team environment and contribute to organisational planning.
Requirements:
* A tertiary qualification in Peer Work, Community Services, Mental Health, AOD or a related field (desirable).
* A lived or living experience of resettling in a home after experiencing homelessness (essential).
