This role involves collaborating with clients and stakeholders to address their goals, needs, and preferences. As a Clinical Partner, you will work in the community setting, providing evidence-based clinical care to individuals in their homes.
The Role
* Provide high-quality, person-centered clinical care to clients in their homes, adhering to best practices and Aged Care Quality Standards.
* Assess clients' clinical care needs and develop, review, and update comprehensive care plans.
* Collaborate with multidisciplinary teams to achieve optimal client outcomes in the community setting.
* Guide care staff to ensure safe and continuous delivery of quality care, while promoting client autonomy and independence.
Requirements
* Nursing qualification and current registration with AHPRA as a Registered Nurse (Division 1, General).
* Experience in client-focused service delivery in aged and community/home care settings, with a passion for delivering high-quality care.
* Knowledge of the Aged and Home Care Reforms, including Aged Care Quality Standards.
* Excellent communication and collaboration skills, with a commitment to advocacy and person-centered care.
* Ability to work effectively in a team environment, with strong administrative and computer skills.
* A current driver's license and ability to travel between sites and services.
