Role Overview
We are seeking a skilled Community Care Professional to join our team in providing person-centred care to clients in the community.
As a key member of our multi-disciplinary team, you will be responsible for assessing, planning, and implementing client care within your scope. This will involve considering physical, social, and psychological requirements, and creating and updating care plans to reflect developing needs.
You will work collaboratively with other team members to ensure clinical quality compliance standards are met. Your day-to-day activities may include:
* Completing scheduled community visits to deliver high-quality care to our clients.
* Developing and maintaining relationships with clients and their families to build trust and confidence.
* Collaborating with other healthcare professionals to ensure seamless care delivery.
Requirements
To succeed in this role, you will need:
* Full, unrestricted registration as a Registered Nurse with AHPRA and unlimited work entitlements.
* General nursing experience post-registration, with a strong understanding of community care principles and practices.
* Excellent communication and clinical problem-solving skills, with the ability to work autonomously.
* A current Australian Driver's Licence and access to a reliable insured vehicle for travel between client locations.
* Confidence in managing your time effectively and prioritising multiple tasks and responsibilities.
