We are actively seeking a full-time Client Care Coordinator to join our team in the Illawarra region. If you have experience in supporting teams, providing client care, and working within an Active Support model, this could be the perfect role for you. You will work closely with the Client Care Manager, Team Leaders and Active Support Workers across 4 of our homes located at Shell Cove, Coniston, Albion Park and Shell Harbour.

About the role
As a Client Care Coordinator at Forsight, you will play a pivotal leadership role in delivering person‐centred, high‐quality care that promotes dignity, independence, and respect for our clients. Leading, mentoring, and managing a dedicated team of active support workers, you will guide their daily activities and ensure that care is tailored to meet the unique needs of each client. In this role, you will oversee the operational aspects of the home, including rostering, resource allocation, and staff management, while fostering a culture of continuous improvement and operational excellence. By ensuring compliance with NDIS standards and addressing client concerns promptly, you will advocate for the overall health and well‐being of those in your care. Collaborating closely with families, guardians, and external service providers, you will facilitate a coordinated approach to client care. Through your leadership, you will cultivate a positive, empowering environment for both clients and staff, strengthening team dynamics and ensuring high performance.

Qualifications
* A Certificate IV in Disability, or a related discipline, or at least 4 years of experience in a similar role.
* A strong background with a proven track record in leading teams in a care environment, with experience in providing feedback, handling performance management, and ensuring adherence to service standards.
* Proven experience in motivating teams.
* Strong understanding of NDIS requirements and the NDIS Code of Conduct.
* Excellent interpersonal and communication skills, with the ability to build positive relationships with clients, families, and team members.
* Strong organisational skills, with experience in rostering, workforce management, and budget management.
* Experience in working with clients with challenging behaviours within a supportive care model.
* A current First Aid and CPR Certificate.
* A valid and unrestricted NSW driver's licence.
* Unrestricted right to work in Australia.
* A current NDIS Worker Screening Check (or willingness to obtain one).
