Key Responsibilities
* Coordinate and manage daily business affairs for the Advanced Illness Management department.
* Lead a team of AIM Clinical Coordinators, NPs, and RNs in delivering exceptional patient care.
* Ensure scheduling, frequency adherence, clinical coordination, and marketing functions meet patient and clinician needs.
* Manage incoming calls from partners and patients, expediting referrals and discharges as needed.
* Oversee lab orders, DME, imaging, and medical record maintenance.
* Coordinate results with NPs, enter telephone encounters in partner EMRs, and reconcile medications.
* Streamline transitions to HSP referrals and identify eligible patients.
About the Role
* Active RN license to practice in the state without any board action.
* Registered Nurse with Home Health, Hospice, and Palliative Care experience preferred.
* Bachelor's degree in nursing or Business Administration required; Master's degree preferred.
* Minimum 2 years health care administration/operations management experience required.
* Certified in Hospice and Palliative Care - preferred.
