Community Support Specialist
Our organization seeks a highly motivated Community Support Specialist to provide vital community-based outreach and recovery-oriented support for individuals aged 16 and over. This role will play a crucial part in developing and monitoring individualized recovery plans that empower consumers to achieve their personal goals.
About The Role
The Community Support Specialist will work closely with the Clinical Lead and Service Manager to ensure that the most appropriate and effective services are provided to consumers of the service.
* Assist in delivering individual assistance for consumers with a mental illness, based on individual recovery goals to support consumers.
* Collaborate with the Clinical Lead/carers/family members/clinicians/community organizations to monitor and review consumer recovery plans.
* Promote positive self-care strategies, enhance social and daily living skills, increase vocational capacity, and facilitate community integration.
* Complete risk assessments and safety planning activities as required for consumers engaged with the service.
Essential Criteria
* Availability to work AM & PM shifts between Monday to Fridays.
* Minimum Certificate IV in Peer Mental Health, Mental Health or Community Services or related field or equivalent.
* At least two years of experience working with consumers with severe and persistent mental illness.
* NSW full Driver's License.
* Understanding of the Principles of Recovery-Oriented Mental Health Practice.
* Demonstrated ability to develop Individual Support Plans and utilize outcome measures.
