The Preventative Integrated Care Service (PICS) will provide a rapid response (review within 24-48 hours) for intensive sub-acute medical management, delivered by a specialised medical, nurse practitioner and allied health team with a disease specific focus through flexible and varied modalities.
• Temporary Part Time opportunity to December 2026 - 64 hours per fortnight.
About the role
This service addresses a gap for a target population of high-risk chronic condition patients requiring specialist intervention and coordinated care in their most vulnerable periods to avoid unplanned admissions and including the 14 days post discharge from hospital to prevent possibly preventable readmissions (PPH).
The Clinical Nurse Consultant (CNC) is a senior Registered Nurse who will provide clinical nursing leadership within the Preventative Integrated Care Service (PICS).
The position provides expert contemporary Diabetes knowledge and skills using contemporary evidence based clinical practice to guide high quality, safe clinical services cross West Moreton Health. This position is also accountable/responsible for the clinical triaging and referral co-ordination for the PICS model of care and will be required to work collaboratively in consultation with the Nurse Practitioners and Senior Medical Officer's to ensure protected delivery of nursing care to chronic condition patients who are accepted to the PIC service.

Our ideal candidate will be someone who can demonstrate the following:
- Contemporary clinical knowledge in the areas of specialised sub-acute and chronic disease management, in a community setting in collaboration with individuals, peers and the healthcare team to achieve improved health outcomes across Preventative Health Services.
- Leadership in the coordination of nursing and provision of health services across the continuum of care, delivering competent nursing care in complex situations and ability to coach patients to adopt self-management strategies.
- Ability to adopt digital technology and utilise digital care solutions and telehealth as part of delivering and co-ordinating care closer to home.
- Advanced level delivery of interpersonal (problem solving/conflict resolution), communication (verbal/written) and time management skills whilst demonstrating autonomous, sound, high level conceptual and critical thinking capabilities.
- Exemplifies the West Moreton Health Professional Practice Model by demonstrating person-centred care, professional excellence, safe and effective care delivery, commitment to quality and safety, and collaborative practice.

- Appointment to this position requires proof of qualification and registration or membership with the Australian Health Practitioner Regulation Agency (Ahpra). Certified copies of the required information must be provided to the appropriate supervisor/manager, prior to the commencement of clinical duties.
