Clinical Pharmacist for Population Health
This role will lead the development and implementation of medication management and support value-based care quality activities across the medical group.
* Key Responsibilities:
* Optimize medication-related outcomes for patients within the medical group by providing direct support for care coordination and transition planning.
* Develop clinical pathways and protocols to improve population health goals.
Requirements:
* Graduate of an accredited school of pharmacy and licensed in the state.
* Prior experience in pharmacy practice, preferably in an ambulatory setting.
* Experience in program development, implementation, and evaluation.
