About the Role
We are seeking an experienced and passionate Registered Nurse to join our Home & Community Clinical team to enable our clients with more complex health needs to achieve their health care goals and continue living in their own home.

Day to Day Responsibilities:
* Providing direct clinical nursing care to our clients in their homes in line with best practice nursing procedures
* Conducting clinical assessments, dressings, medication assessment and catheter changing
* Developing a holistic nursing care plan in collaboration with client, carer, Care Advisor and Allied health where relevant
* Providing safe, quality nursing practice to clients with specific needs and/or complex needs eg. Wound care, palliative care, education and support
* Liaising with all parties involved in the care of the client to maintain holistic health care and wellbeing
* Ensuring all documentation is accurate and completed in a professional and timely manner
* Working in a safe manner to protect yourself, your colleagues and clients closely following all policies and procedures
Requirements:
* A genuine passion for providing high quality care to the elderly
* Current AHPRA registration following completion of a Bachelor of Nursing or similar
* Minimum 3 years post graduate nursing experience
* Experience in clinical assessments
* Initiative and the ability to work independently
* Strong computer skills especially Microsoft Office products and ability to pick up new systems
* NSW Drivers license and a reliable vehicle
* Willingness to apply for a Police Check
