We are seeking an experienced Onsite Aged Care Pharmacist to work within a local residential aged care facility (200+ bed band), supporting medication safety, quality use of medicines, and clinical governance.
Key Responsibilities
* Conduct onsite clinical pharmacy services aligned with the ACOP model
* Medication reviews, audits, and quality improvement activities
* Clinical support and education for nursing and care staff
* Liaison with GPs and the multidisciplinary team
Essential Requirements
* AHPRA-registered Pharmacist
* Accredited with an appropriately recognised ACOP course
* Experience or strong interest in aged care
* Excellent communication and organisational skills
