Making a meaningful impact on individuals' health and wellbeing requires a holistic approach. As a Patient Care Advocate, you will work collaboratively with care coordinators to address diverse social needs influencing health outcomes.
About the Role
* Take a proactive approach to help individuals overcome barriers related to finances, transportation, housing, legal issues, and social isolation.
* Collaborate with participants and their families to create tailored support plans, focusing on person-centred care and collaboration.
* Forge strategic partnerships across diverse organisations to help participants achieve their health and wellbeing goals.
Selection Criteria
* Relevant qualifications in Community Services, Human Services, Social Work, or a Health-related field.
* Experience advocating for and connecting participants with local services (aged care, NDIS, community services).
* Expertise in collaborative working practices and building effective relationships with participants and their families.
* Understanding of the wider determinants of health – social, economic, and environmental factors.
* Strong networking skills and ability to build partnerships across diverse organisations.
