The Aboriginal Youth Empowerment Program aims to offer culturally safe, trauma-informed, and individualised case management, mentoring, and goal setting for disengaged or at-risk Aboriginal youth.
We are seeking a motivated and committed person with formal qualifications in social services, experience in similar case management, and a demonstrated ability to collaborate with staff members, community, government agencies, and stakeholders.
* Promote the Aboriginal Youth Empowerment Program to service providers, stakeholders, and community across the Central Great Southern region.
* Implement and deliver intake referral processes with community.
* Provide initial and ongoing assessment of client needs, including risk assessment and case planning.
* Deliver a strengths-based program focused on personal growth, emotional resilience, health, wellbeing, and employment outcomes.
