Job Details
* Part time (32 hours per fortnight)
* Social Worker Grade 2, Year 1‑4 (SC21‑SC24)
* Location: Caulfield Hospital
The Role
* Provide high‑quality assessment and management of patients within the ABI Community Rehab Service and Transitional Living Service, primarily in the areas of individual and family counselling including grief counselling, adjustment to illness, risk management, coordination of interdisciplinary family meetings and complex discharge planning.
* Support patients' rehabilitation following a brain injury, including goal setting and attainment, and continue support when they are home after discharge.
* Contribute to an interdisciplinary team approach to achieve optimal patient care and minimise clinical risk.
* Contribute to quality improvement and research activities within the ABI service.
* Promote a culture of ongoing learning and development within the team.
* Enhance the overall functioning of the ABI programs by participating in necessary administrative tasks.
Qualifications and Experience
* Recognised degree in Social Work.
* Eligibility for membership of AASW.
* Previous experience in a community rehabilitation setting (highly desirable).
