Child and Family Practitioner Role
The role involves working with foster carers to provide high level support and supervision in caring for children and young people.
This includes developing and implementing individualised care plans in partnership with relevant government agencies, children and their families, carers and other stakeholders.
Key Responsibilities
Develop and implement care plans: Work closely with foster carers, children, families and stakeholders to develop and implement tailored care plans that meet the unique needs of each child.
Provide high level support: Offer expert guidance and support to foster carers to ensure they are equipped to provide the best possible care for children.
Collaborate with stakeholders: Work collaboratively with government agencies, health professionals and other stakeholders to ensure a holistic approach to supporting children and families.
Monitor and evaluate progress: Regularly monitor and evaluate the effectiveness of care plans and make adjustments as needed.
