Type: Full-time 38hrs or part-time 30hrs/pw (WFH, some driving required)
At Country Home Services, we've been supporting older people in our regional communities for over 35 years, providing care that's personal, respectful and truly makes a difference. Country Home Services is expanding service offerings under the new Support at Home (aged care) program. We are looking for qualified Care Partners who are compassionate, organised, and committed to helping older people live well at home.
As a CHS Care Partner, you'll be the first point of contact for clients, working closely with them and their families to ensure their care aligns with their personal goals, preferences, and changing needs. Your enthusiasm and commitment to learning and adapting in line with Support at Home reforms and the Commonwealth Home Support Program (CHSP) will be highly valued as you and your team work together to deliver high-quality, person-centred care services to clients within our regional service areas.
Key responsibilities:
* Coordinate services, advocate for client wellbeing, and contribute to a safe and supportive care environment
* Build meaningful relationships with clients and develop individualised care plans
* Support independence, wellbeing and choice through person-centred care
* Liaise with home care workers, families, contractors and health professionals
* Keep accurate and up-to-date care and budget records within Visual Care client care system
* Contribute ideas and feedback to improve our services
What you'll bring:
* Certificate IV or Diploma in Community or Aged Care, Nursing or related qualification (essential)
* Experience in aged care, health or community services
* Strong communication, teamwork and problem-solving skills
* Solid computing skills (Word, Excel and databases) and basic budget management skills
* Current driver's license and a reliable and registered vehicle
* Senior First Aid/CPR or willingness to obtain
* A valid National Police (Aged Care) or NDIS Check
