Job Overview
We are seeking a pro-active Social Worker/Specialist Support Coordinator to join our team.
* Identify and address barriers to implementing client's NDIS plans.
* Build relationships with clients, family, disability services organisations, clinical staff, and government departments.
The role requires collaborative approach to ensure supports are delivered effectively.
Key Responsibilities
Assist in building a foundation for best practice care, staff training, support, and intervention for clients.
* Develop and implement comprehensive support plans.
* Work closely with multidisciplinary teams to achieve positive and sustainable outcomes.
Experience working with clients with dual diagnoses is essential.
Requirements
To be successful in this role you will need:
* Bachelor's degree in social work or other relevant discipline.
* Demonstrated communication skills and ability to work independently and collaboratively as part of a team.
* Organisational skills, time management, and prioritisation.
* Computer literacy in Microsoft Office.
