About the Organisation
Sunrise Health Service Aboriginal Corporation has operated since 2003 and is the second largest Aboriginal Community Controlled Health organisation in the Northern Territory.
Based in Katherine, Sunrise Health Service services 9 remote communities east of Katherine. We serve a population base predominantly Aboriginal, of approximately 3,800 people - many with chronic illnesses requiring early detection and amelioration.
About the Prospect
The Elder Care Connector (ECC) is responsible for engaging with local Elders, older Aboriginal and Torres Strait Islander people, and their families to raise awareness of aged care entitlements and services.
This role involves active community outreach to ensure clients can access both internal and external aged care services. The ECC will work closely with the Elder Care Support Coordinator to identify local demand, assess current service availability, and address service gaps.
In addition to connecting clients with appropriate services, the ECC will advocate on behalf of individuals, ensuring their needs are met within the aged care system. This position requires building strong relationships within the community, ensuring that cultural considerations are respected, and fostering trust so that Elders feel empowered to seek support when needed.
The ECC will also help to promote aged care services and raise awareness about available support, contributing to the overall well-being of older community members.
About You!
- Identifies an Aboriginal person; recognised and accepted as an Aboriginal person by the Aboriginal community
- Strong communication and interpersonal skills, with the ability to build trusting relationships within the community.
- Proficiency in or knowledge of local dialects or languages spoken within the community.
- A demonstrated understanding of person-centred care and trauma-informed practices.
- Experience working independently and as part of a team.
- Commitment to cultural awareness and sensitivity, with the ability to work respectfully with Aboriginal and Torres Strait Islander communities.
- A willingness to learn about the aged care system and the services available to clients.
- Ability to enter data and maintain accurate records for reporting purposes.
Desired Skills and Qualifications:
- A qualification in Community Services and/or Aboriginal Mental Health and First Aid.
- Previous experience working with older people, particularly within the aged care sector.
