* The ideal candidate will have experience in conducting assessments, developing and delivering support plans, and working collaboratively within a multidisciplinary team.

Key Responsibilities

* Assist participants, families, and carers in identifying needs through assessment and person-centred planning.
">
* Evaluate, develop, and deliver evidence-based support plans within complex support spaces.
">
* Work collaboratively within a multidisciplinary team to ensure individual plans reflect participants' goals, passions, and strengths.
">
">
Requirements
">
* 2+ years of NDIS experience
">
* Current registration with AHPRA as an Occupational Therapist
">
* Active ABN
">
* Relevant insurance, including Public Liability, Professional Indemnity Insurance, and Comprehensive Car Insurance
">
* Police clearance, WWC, and NDIS screener
">
