Integrated Care Coordinator Position

We are seeking an experienced Integrated Care Coordinator to join our multidisciplinary team at Nepean Blue Mountains Local Health District. The successful candidate will provide clinical leadership and expert care coordination support to consumers of our service.
The position involves working collaboratively with healthcare professionals, consumers, and carers to develop and implement individualized care plans that promote recovery, well-being, and quality of life.
To be successful in this role, you will possess strong clinical skills, excellent communication and interpersonal skills, and the ability to work effectively in a fast-paced environment.
The ideal candidate will have experience working in mental health, post-registration qualifications in Mental Health, and at least 3 years' experience in a similar role.
The successful applicant will also possess organizational skills, the ability to manage complex caseloads, and high-level written and verbal communication skills.
