About the role
We are seeking a Case Manager to join the Senior Community Care team to provide efficient and effective case management services to clients of Seniors Community Care Services. This role assists older people to remain living safely and independently in their own home and community by facilitating and coordinating the provision of care arrangements that are suitable for their individuals needs.
This role is based at our Somerville office.
As our ideal candidate, you will possess:
* Minim Certificate III in Aged Care/ Community Services or equivalent
* Knowledge and demonstrated capacity to work with resources relevant to Aged Care and Community Care
* Demonstrated understanding of appropriate documentation processes and maintain all documentation to a high standard
* Case Management level experience in Heal and Aged, or Home Care/Community Services
* Well- developed interpersonal and communication skills
* Understanding and experience in working with quality improvement systems in Aged Care
* A high level of organisation and time management skills
