About the Role
The Case Manager will work with young people aged 15-25 to support their transition from care to independent living. This involves developing a personalized plan to achieve their goals and aspirations, and connecting them with relevant community resources.
Main Responsibilities
* Work with young people to create comprehensive plans for transitioning to independent living
* Utilize therapeutic crisis intervention techniques to assist in recognizing and de-escalating challenging behavior
* Nurture relationships with significant family members, friends, community organizations, and government agencies
* Incorporate strategies within assessments and goal plans to help young people maintain their cultural identity and heritage
* Support and promote engagement with local Aboriginal and Torres Strait Islander Communities and Community Groups
Requirements
* Relevant tertiary qualifications (3-year bachelor's degree) in the human services sector; social sciences, social work, psychology, or related fields
* A current QLD driver's license
* Demonstrated experience working with young people, supporting them to articulate their goals, strengths, and aspirations
* Demonstrated experience engaging mainstream services (housing, education, health, mental health, and social support)
* Demonstrated experience managing complex caseloads and maintaining accurate documentation
