Care Coordinator Role Available for RNs in Community Disability Setting
At National Networks Plus, we pride ourselves on providing a unique staffing solution to the Aged Care and Community Care sectors. We are currently seeking a dedicated Registered Nurse to join our team as a Care Coordinator in a community disability setting.
About The Role
You will be responsible for managing the complex health needs of residents living with physical and intellectual disabilities across two supported accommodation houses in Woy Woy. This role requires a compassionate and experienced RN who can provide person-centred care, oversee medical and clinical management, and support care staff with personal care tasks.
Key Responsibilities:
* Medical and Clinical Management: Oversee epilepsy management, catheter care, PEG feeding regimes, dysphagia support, and adherence to Modified Meal Plans (MMP) for oral intake.
* Care Support: Assist care staff with personal care tasks and monitor resident health status changes.
Required Skills and Qualifications
To be successful in this role, you will require:
* A current AHPRA registration as a Registered Nurse.
* Experience working in disability or aged care settings.
* A strong understanding of PEG, catheter, and seizure management.
* Excellent communication skills, especially when working with non-verbal clients.
* A compassionate, patient, and team-oriented approach.
