Integrated Case Management Specialist
As an Integrated Case Management Specialist, you will provide professional casework services to young people subject to a youth justice intervention.
You will demonstrate cultural capability and/or a commitment to developing cultural capability to ensure an inclusive culture that aligns with the Human Rights Act 2019 and supports departmental programs and community partnerships aimed at preserving cultural connections for Aboriginal and Torres Strait Islander peoples.
* Deliver high-quality casework services to young people using advanced assessment and intervention skills to deliver integrated and culturally appropriate services.
* Prepare and present information and evidence to the Children's Court, including conducting pre and post-court interviews, liaising with key stakeholders and preparing reports and other documentation as required.
* Engage with internal and external stakeholders and service providers to achieve quality casework outcomes.
Key responsibilities include providing effective coordination of therapeutic alliances, formulating case plans with consistent action-oriented goals to support young people and their family members. This may involve assisting parents/guardians, siblings and/or young people to access employment, education, physical and mental health services, safe housing, learn responsible parenting skills and address anti-social behavior.
Additionally, you will undertake record and information management tasks, including maintenance of case records, preparation of reports and data entry to ensure accountability in accordance with policy and procedures.
Furthermore, you will collaborate with Youth Justice Service Centre staff on complex cases, program development, exercising of statutory delegations and other matters as required.
As a specialist, you will participate in regular supervision, apply self-reflective practices and contribute to peer supervision. You will also take on a leadership and mentoring role to less experienced staff, including supervision of Youth Workers involved in the Integrated Case Management Program.
