About the role
Support children and young people in Out-of-Home Care through case management, therapeutic planning, and frontline coaching. Drive positive outcomes by embedding trauma-informed, culturally protected practices and ensuring compliance with standards and legislation.
The role operates across two primary functions:
- 18 hours per week supporting frontline staff in the homes to implement case and care strategies.
- 20 hours per week completing case work duties, including planning, documentation, and stakeholder engagement.

What You’ll Be Doing
- Lead and review case plans to meet therapeutic and cultural goals
- Coach and support staff to embed strategies in daily care
- Engage with families, carers, and agencies to align goals
- Maintain documentation and ensure compliance with standards
- Promote safe, nurturing, and culturally respectful environments
What You’ll Bring
- Diploma (or working towards) in community, youth, or social work
- Proven case management and stakeholder engagement skills
- Knowledge of trauma-informed and therapeutic care practices
- Strong documentation and compliance capability
- Current driver’s licence and probity clearances
