About the Role
We are seeking an experienced Community Nurse to provide high-quality holistic care in Aged, Disability, Transitional & Palliative Care and Hospital in the Home.
Your Key Responsibilities
* To conduct client assessments, clinical care and reviews to ensure comprehensive and personalized support
* To interpret health, aged and disability care policies and regulations for carer and nursing staff
* To ensure compliance with local, state, and federal regulatory agencies related to aged and disability care services
* To implement and maintain operating systems including Visual Care, policies & procedures & service conditions for caregivers, nurses & other care professionals
* To encourage on-going professional development of the Care Team and promote 'best practice' standards
* To serve as a liaison with the accounts department, ensuring proper billing in line with the care plan
* To comply with OH&S regulations and ensure a safe working environment
* To meet with clients and their families, assessing their needs and building strong connections
* To conduct client care assessments and prepare care plans to best meet client requirements
* To provide some nursing care and in-home care services as needed
* To handle information requests, feedback, and general enquiries
* To ensure all documentation is accurately and comprehensively completed in Right at Home databases and software programs
Requirements
* A Registered Nurse (RN) with 1-2 years of experience in community nursing and current AHPRA registration
* An alternative qualification, such as Certificate III or IV in Aged/Disability Care, with a minimum of 4 years' experience
* Proven experience in aged care, disability, or mental health within a community setting
* A strong understanding of Aged Care and Disability Standards, as well as consumer-directed care principles
* Current First Aid, CPR certification, and National Police Check
* A valid driver's license and access to a reliable car with comprehensive insurance
