Role Overview
This is an exciting opportunity to contribute to the delivery of robust assessments, crisis response and specialist FV case management services to victim survivors.
The role will be instrumental in delivering high quality, safe and effective family violence and child well-being responses to those seeking support, safety, and recovery.
* Establish effective relationships with clients that encourage self-determination and participation and uphold the principles of the National Family Violence Model of Care.
* Provide specialist FV case management and crisis support to clients in accordance with TSA case management models and standards of practice.
* Make ongoing assessment of the victim survivor's issues, needs and circumstances from initial intake to exit.
* Conduct ongoing family and domestic violence risk assessment and safety planning with clients using agreed tools.
* Develop individual case plans in collaboration with victim survivors and include a range of interventions that address the needs or goals of clients.
* Implement and actively lead case plans by the victim survivor.
