Aboriginal Liaison Officer - Chronic Disease Job Description
This role supports clients by promoting early intervention, self-management, and culturally safe care pathways for those living with chronic illness. Working collaboratively with hospitals, community health, aged care, and Aboriginal Community Controlled Health Organisations (ACCHOs), you will play a vital role in improving care coordination and reducing unnecessary hospital visits.
As the successful applicant, you will:
* Advocate for Aboriginal patients across acute and emergency settings.
* Collaborate with chronic disease clinicians to promote culturally appropriate care and education.
* Support clients and families in developing and engaging with goal-oriented, person-centred care plans.
* Foster partnerships between SWH, ACCHOs, and other service providers.
* Provide tailored health education and encourage active participation in self-management programs.
* Assist with capacity building across SWH by participating in education, training, and quality improvement initiatives.
About the Role
The successful applicant will work independently and within a collaborative team environment, providing strong skills in assessment, care planning, and service delivery. The ideal candidate will have relevant qualifications in Nursing, Allied Health, or Aboriginal Health, with current AHPRA registration (as applicable).
