Key Responsibilities
The Older Persons Community Mental Health Service provides specialist mental health assessment, diagnosis, care coordination, and treatment for individuals aged 65 and over, as well as younger adults with complex age-related needs.
* Providing high-quality, person-centered care that is recovery-focused and trauma-informed.
* Demonstrated skills in safe and effective triage, assessment, risk assessment, and care planning.
