About the Role:
* Provide clients with the assistance, care and support they have been individually assessed and reflected in their individualised support plan.
* Provide domestic assistance, personal care, and support services.
* Liaise with families, relevant professionals, Home Support Partners and other stakeholders as required.
* Escalate immediately any client health and wellbeing concerns to the Service Delivery Team Leader.
* Perform other duties as directed, commensurate with the skills and expectations of the role.

About you:
* Ability to respectfully maintain our clients' right to dignity, privacy & dignity of choice.
* Currently holds a Certificate III in Individual Support (Ageing) or equivalent qualification, with knowledge and understanding of the CHSP program and reform agenda.
* Hold a current unrestricted ACT Working with Vulnerable People card and
* Hold current First Aid and CPR Certificate.
* Current Drivers licence (minimum Green Provisional or Full Australian Licence)
