Permanent Full Time - DarwinApply now Job no: 505192Employment type: Full TimeLocation: DarwinCategories: Aged CareAbout the RoleAs a Care Coordinator you will oversee and enhance the delivery of our Aged Care services in Darwin. Some of your duties will involve processing referrals, aiding clients in commencing with our services, and continuously adjusting care plans to suit evolving needs. You will coordinate home visits to ensure the needs of the people we support are met and that we are working in partnership with them and their families.Additionally, you'll contribute to program coordination by assessing, improving, and overseeing operational and administrative systems. You will deliver exceptional service to ensure our clients and their families feel valued and supported, while also fostering the growth of our aged care services.This is a Permanent Full Time opportunity based in our Darwin office, with some travel required to visit clients in their homes.Key ResponsibilitiesUndertake comprehensive client assessments while developing client profiles and care plans in collaboration with the client/representative and other relevant stakeholdersEnsure care plans are monitored and reviewed on a regular basis to align with the changing needs of clients, including regular client home visitsMonitoring the clinical risks and needs of a client, ensuring referral pathways are establishedCollaborate effectively with internal stakeholders to ensure clients’ needs are met including our Rostering team and Workforce LeadersDevelop strategies and/or broker services which will address the identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communitiesRepresent Life Without Barriers at external forums and meetings, promoting the organisation within the communitySkills & ExperienceDemonstrated Aged Care or other clinical case management experienceUnderstanding of Home Care Packages and Commonwealth Home Support ProgramHigh level competency in utilising a variety of technologyAbility to identify and implement risk mitigation strategiesStrong communication skills to build relationships with diverse stakeholdersAbility to multi-task and work autonomously in a fast-paced environmentA current unrestricted Australian Driver Licence and reliable vehicleThorough three-day induction programCareer pathways across a national organisationAttractive remuneration package with a monthly rostered day off (Think 12 extra long weekends a year!)Join a supportive remote working team that are passionate about the work they doUp to $15,900 of your annual salary tax-free through Salary Packaging, increasing your take-home payAccess to Fitness Passport benefitsSuccessful candidates will be required to clear probity checks including National Criminal History Record Check and complete a statutory declaration.How to ApplyInclude your resume and covering letter in one document, click ‘Apply’ and follow the prompts.
