About the role
As a Social Worker with the NWHHS, you will play a key role in supporting patients and families through a holistic, person-centred approach to care. You'll collaborate with multidisciplinary teams across a range of clinical settings to provide high-quality, culturally responsive, and evidence-based services that enhance patient wellbeing and recovery.
In this role you will:
* Provide Social Work and case management services within a multidisciplinary team across varied clinical areas.
* Undertake psychosocial assessments, discharge planning, counselling, and crisis intervention.
* Participate in weekend rosters, on-call, extended hours, and outreach services as required.
* Support a safe, patient-centred culture and uphold high standards of clinical care.
* Build skills through supervision, ongoing learning, and quality improvement activities.
* Deliver culturally safe, responsive care for Aboriginal, Torres Strait Islander, and diverse communities.
Requirements
* Mandatory requirements are a relevant tertiary degree qualification and appropriate registration/licences.
* Pre-employment screening will be conducted for the successful applicant, including Criminal History checks.
* This is a VPD risk role and may require evidence of vaccination or immunity to: Hepatitis A / B, Measles, Mumps, Rubella, Varicella, Pertussis, and Tuberculosis.
* Potential applicants are advised that the Commission for Children and Young People and Child Guardian Act 2000 requires Queensland Health to seek a 'working with children check; from the Commission for Children and Young People and Child Guardian prior to appointment to this position.
* A period of probation may apply.
