Senior Renal Social Worker
We are seeking an experienced Senior Renal Social Worker to join our multidisciplinary team. The successful candidate will provide high-level psychosocial support, assessments, counseling, and advocacy to patients with renal disease.
Key Responsibilities:
* Provide specialist social work services to consumers and their carers.
* Develop and implement treatment plans to optimize health outcomes.
* Collaborate with multidisciplinary teams to ensure coordinated care.
Requirements:
* A Social Work degree that provides eligibility for membership of the Australian Association of Social Workers (AASW).
* Experience working in a health setting with patients managing complex health conditions.
* Strong communication and interpersonal skills.
* Ability to work effectively in a fast-paced environment.

What You'll Be Doing
This position is responsible for providing specialist social work service to consumers and their carers to ensure the provision of optimal health outcomes consistent with policies and procedures, standards, relevant legislation, and the NSW Health Code of Conduct.
The position will also participate in professional development, clinical supervision, teaching, quality activities, and other relevant departmental and administrative duties.
