Senior Community Care Coordinator
We are seeking an experienced Senior Community Care Coordinator to join our team and make a positive impact on the lives of our clients.
The ideal candidate will have a strong background in aged care, case management, and community services. They will be responsible for developing and implementing personalized care plans, coordinating multidisciplinary support teams, and advocating for clients' needs.
* Main Responsibilities:
* Develop and implement client-centered care plans that promote independence and well-being
* Coordinate and manage daily care delivery, including individual budgets and support clinical pathways
* Act as a Restorative Care Partner and contribute to the Support at Home program
* Provide clinical support and nursing care during scheduled appointments
Requirements:
* Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA)
* Proven experience in case management, preferably within community or aged care settings
* Tertiary qualifications in Aged Care Case Management highly regarded
* Skilled in HCP assessments with a holistic and goal-oriented approach to care
* Confident communicator with strong networking abilities across internal and external stakeholders
* Hold a valid Australian driver's license and be comfortable travelling locally for client visits
