About the Role
We are looking to appoint a Grade 1 Social Worker in a full‑time (1.0 EFT) ongoing capacity, to work on the Monash Heart, Cardiothoracic and Ambulatory Care units at the Victorian Heart Hospital (VHH), part of Monash Health.
As a Social Worker, you will triage new referrals and meet with VHH patients during their treatment onsite, or in the outpatient setting. The focus will be on conducting psychosocial assessments, risk assessments, providing emotional support and working collaboratively with relevant parties.
You should have a sound understanding and working knowledge of the range of service systems including Family Violence, Child Protection, My Aged Care, Transition Care Program, the National Disability Insurance Scheme (NDIS), Mental Health and the recovery model to ensure we provide high‑quality, evidence‑based social work service to all patients and families.

You are confident undertaking complex psychosocial assessments and managing an active clinical caseload. Qualification & Licence Requirements
* Bachelor of Social Work or Master of Social Work Qualifying, or Master of Social Work, or equivalent as assessed and approved by the Australian Association of Social Workers (AASW)
* A current Victorian Driver's Licence
