Healthcare Delivery Lead

The primary objective of this role is to coordinate and execute the delivery of high-quality healthcare services to a prison population. As a Senior Nurse - Primary Care, you will work closely with the multidisciplinary healthcare team, providing expert guidance and mentorship to ensure optimal care delivery.

This position requires strong clinical assessment, care planning, and problem-solving skills, as well as knowledge of quality improvement and evidence-based practice in healthcare. The ideal candidate will have experience working in a primary nursing setting, preferably within a forensic or correctional environment.

The successful applicant will possess demonstrated leadership skills and the ability to manage and mentor a multidisciplinary team. They will also be committed to ongoing personal and professional development, and have a thorough understanding of legislation related to Equal Opportunity, Disability Services, and Occupational Health & Safety.

In this role, you will be responsible for:

* Coordinating and managing the provision of primary healthcare services to inmates
* Supporting clinical assessments, care planning, and discharge processes
* Providing mentorship and guidance to new staff and ensuring best practice standards
* Collaborating with other healthcare professionals to deliver quality services
* Assisting in risk assessments, patient support, and harm minimisation strategies
* Supporting quality improvement projects and participating in policy development
* Maintaining comprehensive clinical documentation and adhering to facility requirements

Key Responsibilities


1. Healthcare Service Coordination

Coordinate and manage the provision of primary healthcare services to inmates, ensuring timely and effective delivery of care.


2. Clinical Support and Guidance

Support clinical assessments, care planning, and discharge processes, providing expert guidance and mentorship to new staff.


3. Leadership and Team Management

Lead and manage a multidisciplinary team, providing mentorship and guidance to ensure best practice standards.


4. Collaboration and Quality Improvement

Collaborate with other healthcare professionals to deliver quality services, supporting quality improvement projects and participating in policy development.


5. Risk Assessment and Patient Support

Assist in risk assessments, patient support, and harm minimisation strategies, ensuring comprehensive clinical documentation and adherence to facility requirements.


Requirements

To be successful in this role, you will need:

* Current registration as a Registered Nurse – Division 1 with the Australian Health Practitioner Regulatory Agency (AHPRA)
* Strong clinical assessment, care planning, and problem-solving skills
* Knowledge of quality improvement and evidence-based practice in healthcare
* Experience working in a primary nursing setting, preferably within a forensic or correctional environment
* Demonstrated leadership skills and the ability to manage and mentor a multidisciplinary team
* A commitment to ongoing personal and professional development
* Understanding of legislation related to Equal Opportunity, Disability Services, and Occupational Health & Safety
