The Role
As our Care Partner, you will play a pivotal role in shaping personalised care journeys for our clients. This role is about more than coordination—it's about making a meaningful difference. You will:
* Act as the key contact and advocate for clients across Support at Home (transitioning from Home Care Packages), Fee for Service, Veterans Home Care and Brokerage programs
* Develop and manage holistic care plans in line with the new Aged Care Act 2024 and Strengthened Quality Standards
* Work closely with Clinical Team to achieve outstanding wellness and reablement outcomes
* Optimise quarterly budgets through thoughtful planning and innovative service solutions
* Promote client choice, independence and dignity, ensuring rights are always respected
What You'll Bring
* Demonstrated experience in aged care coordination, ideally within in‐home services
* Certificate IV in Aged Care/Community Services and/or a relevant tertiary qualification (clinical background highly regarded)
* A genuine passion for improving the lives of older Australians
* Strong organisational skills, attention to detail and a reliable work ethic
* Current National Police Clearance (issued within the last three months)
* Australian citizenship or a valid visa with working rights
* Current Australian Driver's Licence
